We do this by carefully planning your transition from the hospital or rehab center and your continued care at home. A report by the CDC states that nearly 20% of people aged 65 and over are readmitted back to the hospital within 30 days.

Four main reasons for readmission are:

  1. Lack of Follow Up: more than half of patients who are readmitted within 30 days did not follow up with their primary care doctor.
  2. Lack of Medication Management: 75% of patients fail to take their medications correctly due to the inability to understand the directions, remember a new medication schedule, or discontinue medications that have been changed.
  3. Lack of Communication: More than 50% of patients do not communicate changes in their condition to their primary care doctor. What could be a simple office visit ends up being a readmission.
  4. Living Alone: A person that lives alone has a 50% or higher chance of being readmitted. This is mostly due to a lack of assistance in the home to pick up food, medications and assist with follow up appointments.

Assisting Hands Home Care caregivers will:

  • Coordinate with your discharge team to ensure proper follow up
  • Do a Home Safety Evaluation
  • Ensure prescriptions are provided and get New Prescriptions
  • Provide Medication Reminders and remove old or unnecessary meds
  • Schedule follow up doctor appointments and help you get to them
